- Gray t-shirt (HMS or plain)

*For current 6th grade students and parents wishing to enroll in Girls Athletics during the 2022-2023 School Year.

**REQUIRED PRIOR TO PARTICIPATION**
Prior to participation each school year, any student enrolled in Athletics, Cheer, or Dance must complete a series of paperwork.
(6th grade students do not require a physical for Pre-Athletics).
Students are not allowed to dress out or participate in any way until ALL forms are complete.
The UIL Physical Form can be found HERE.
All online forms will be published as soon as they are available.
Students and parents must complete the online UIL acknowledgement forms. (please do not print them, they must be online)
You may print the instructions HERE.
Additionally, you must complete a UIL physical with a physician. It must be dated AFTER April 1, 2022 to be valid for the 2022-2023 school year.
